Telehealth Consent

Please read this Telehealth Informed Consent ("Consent") carefully before using Superday's telehealth services. By proceeding with an intake or consultation through Superday, you acknowledge that you have read, understood, and agree to this Consent.

1. Purpose

This Consent explains the nature of telehealth services offered through the Superday platform (operated by MiNood Health Inc.) and provided by independent, licensed clinicians affiliated with DrTelx (the "Provider Group"). Telehealth involves the use of electronic communications to enable clinicians to evaluate, diagnose, consult with, and, where clinically appropriate, treat you without an in-person physical examination.

2. Nature of Telehealth Services

Your evaluation may be conducted synchronously (live) or asynchronously (via questionnaire, written history, and/or photographs, reviewed by a licensed clinician), depending on the service. Clinicians make treatment decisions based on the information you provide. The completeness and accuracy of the information you submit directly affects the clinician's ability to safely evaluate you.

3. Benefits and Risks of Telehealth

Potential benefits include improved access to care, convenience, and reduced travel and wait times.

Potential risks include: the absence of a physical, in-person examination may limit the clinician's ability to detect certain conditions; technical failures may interrupt or delay care; information transmitted electronically carries an inherent, though minimized, risk of a security breach; and in rare cases, the clinician may determine that telehealth is not appropriate for your condition and may refer you to in-person care.

4. Compounded Medication Disclosure

Certain products offered through Superday, including QUATtro™ (a compounded sublingual formulation combining apomorphine, sildenafil, tadalafil, and vardenafil), are compounded medications prepared by FOOTHILLS Professional Pharmacy or an affiliated licensed pharmacy pursuant to a valid prescription and patient-specific clinical need.

You understand and acknowledge that:

  • Compounded medications, and specifically compounded combination products, are not reviewed or approved by the U.S. Food and Drug Administration (FDA) for safety, efficacy, or the specific combination being prescribed;
  • Compounded medications are prepared based on a licensed pharmacy's compounding process and may vary from FDA-approved, commercially manufactured versions of the individual active ingredients;
  • Your prescribing clinician has determined, based on your intake information, that a compounded formulation may be clinically appropriate for you; and
  • You have the opportunity to ask your clinician questions about the compounded medication, its ingredients, and alternatives before proceeding.

5. No Guarantee of Outcome

Superday and the Provider Group make no guarantee regarding diagnosis, treatment approval, or clinical outcome. Your clinician retains full independent judgment and may determine, at any point, that treatment is not appropriate for you and may decline to prescribe or may discontinue treatment.

6. Emergency Disclaimer

Telehealth services offered through Superday are not intended for medical emergencies. If you are experiencing a medical emergency, including but not limited to chest pain, difficulty breathing, an erection lasting longer than 4 hours (priapism), or any other urgent symptom, call 911 or go to the nearest emergency room immediately.

7. Confidentiality

Information exchanged during your telehealth consultation is confidential and is protected consistent with applicable state and federal law, including HIPAA, to the extent applicable, as described in our Privacy Policy. As with any electronic transmission of information, there is a limited inherent risk that confidentiality could be breached by technological failure or unauthorized access.

8. Your Responsibilities

You agree to provide complete, accurate, and truthful information regarding your medical history, current medications, and symptoms. You understand that withholding or misrepresenting information may compromise your safety and the clinician's ability to provide appropriate care. You agree to seek in-person or emergency care when advised to do so by your clinician or when your condition warrants it.

9. Voluntary Consent

Your participation in telehealth services is voluntary. You may withdraw your consent to telehealth treatment at any time by notifying Superday at hello@superday.health, without affecting your right to future care, though withdrawal may result in discontinuation of any active treatment plan.

10. Acknowledgment

By proceeding with your intake and consultation, you acknowledge that:

  • You have read and understood this Consent;
  • You have had the opportunity to ask questions, and any questions have been answered to your satisfaction;
  • You voluntarily consent to receive telehealth services from DrTelx's licensed clinicians through the Superday platform; and
  • You understand the risks, benefits, and limitations described above, including those specific to compounded medications.
